Square Medical Group’s (“Square” or “SMG”) mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of 5,000 across Massachusetts: medication management, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as three levels of housing (i.e. Medicaid reimbursed Co-Occurring Recovery Residences, self-pay-based sober homes, and state-sponsored low threshold housing).

Program Overview

The residential recovery programs are structured rehabilitation-oriented treatment focused on trauma-informed care and harm reduction. It is designed for motivated individuals who want to enhance the quality of their day-to-day lives. Through active engagement in groups and the development of the community, we will work together to overcome challenges that interfere with our abilities to live life to the fullest.
Residents will be assigned a weekly life skill to ensure ongoing cleanliness/maintenance of the house. Residents are expected to take part in assigned life skills. Staff will help if residents are unsure about how to complete a task. Life Skills are necessary to independent living.
On average, residents spend between three to six months in the three phases of the residential program. Each phase of the program rewards residents with increasing levels of responsibilities and freedoms. Throughout the program, residents are supported by deep expertise from SMG’s outpatient services, our core capability. Square Medical Group is looking for a extraordinary Licensed Practical Nurse (LPN) who is an energetic, team player to join our thriving team in our innovative residential program.
Position Summary: You will be working in our "Gayle House" Weymouth Program. It is a 24-bed intensive treatment program for adult women designed specifically for individuals with co-occurring mental health and substance use disorders. We currently need an experienced Licensed Practical Nurse to help monitor medications and medical issues for our residents at both sites! You will join a multi-disciplinary team, working under the direct supervision of the COE Clinical Director with input from the COE Program Director and Medical Director.
Qualifications & Education:
- 3-5 years’ experience in substance use disorder and co-occurring disorder population preferred.
- Active Commonwealth of Massachusetts license - LPN required.
- Strong documentation skills including ability to utilize an EMR required.
- Previous experience working within a multidisciplinary team Active Commonwealth of Massachusetts license - LPN required.
- Strong Communication skills.
Responsibilities:
- Participate in each resident’s Intake and psychiatric assessment.
- Monitor resident medication management.
- Organize, manage and provide oversight for medication room in COE Residence.
- Analyze and evaluate patient information to diagnose co-occurring medical disorders.
- Assist in the development of treatment plans.
- Explains medication effects and side-effects to resident and/or guardian upon admission with review of the Patient Medication information (PMI) for each medication.
- Reviews all non-psychiatric medical orders and prescriptions.
- Participates in and provides input to multi-disciplinary treatment team at Gayle House.
- Orders non-prescription, first aid, drugs (i.e. analgesics, antipyretics) as needed.
- Interact with residents, counseling staff, medical director, and other members of the interdisciplinary care team to promote a high standard of care, discuss treatment plans and progress.
- Revise/update plan of care based on patient progress.
