Work Arrangement: Fully Remote (Optional Hybrid)

Schedule: Monday-Friday, 9a-5p

Location (Optional): Braintree, MA

Position Overview

We are seeking an experienced and compassionate Registered Nurse Case Manager to join our team in a fully remote capacity. This role offers flexibility, with the option for candidates to work a hybrid schedule if they prefer to spend a few days on-site at our Braintree, MA location-though on-site work is not required. The RN Case Manager will play a critical role in coordinating care, supporting patients, and ensuring high-quality outcomes across assigned cases.

Key Responsibilities

• Manage and coordinate patient care plans across the continuum of care

• Conduct clinical assessments and ongoing case reviews

• Collaborate with interdisciplinary teams, providers, and stakeholders

• Advocate for patients by identifying needs, resources, and appropriate interventions

• Ensure compliance with clinical guidelines, policies, and regulatory requirements

• Maintain thorough and timely documentation within case management systems

Qualifications

• Active Registered Nurse (RN) license

• Prior experience in case management or care coordination preferred

• Strong clinical judgment and organizational skills

• Ability to work independently in a remote environment

• Excellent communication and collaboration skills

Work Environment

• Fully remote role with optional hybrid flexibility

• No requirement to be on-site, though access to the Braintree, MA office is available for those who prefer a hybrid schedule

The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

• Current RN licensure in state practicing

• At least one year of Case Management experience preferred

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or state regulation)

• Must meet all federal, state and local requirements

• Must be at least 18 years of age
