Responsibilities
- Provide routine hospice nursing visits (approximately 4–5 visits per day) in patients’ homes and outpatient settings
- Conduct assessment, care planning, symptom management, and patient education in collaboration with the interdisciplinary team
- Administer medications and therapies per physician orders; monitor patient response and adjust plans as needed
- Document all care and communication in the electronic health record; coordinate with team members and families
- Travel between patient locations within the prescribed radius; maintain safety and infection-control standards
- Support families with care planning, discharge planning, and available community resources

Requirements
- Active Massachusetts RN license (must have MA license at time of submission)
- BLS certification
- Experience: Experienced hospice RN; outpatient/home hospice care preferred
- Travel capability: Able to travel 30–50 miles daily; valid driver’s license and reliable vehicle
- Education: Graduation from an accredited nursing program and active RN licensure in MA
- Availability: Start on 06/15/2026 for a 13-week assignment
