We are seeking a Registered Nurse who values thoughtful, relationship-based care and a manageable daily workload. This role offers the opportunity to work closely with patients while providing coordinated nursing care across a variety of local care settings.

You will support patients through assessment, care planning, and follow-up, collaborating closely with a multidisciplinary clinical team. The position blends mostly outpatient, with some field-based care, while maintaining a predictable weekday schedule and reasonable daily visit expectations.

What we’re looking for

• Active Registered Nurse license in Massachusetts
• 2+ years of RN experience in outpatient, community, or care coordination roles
• Comfortable working independently while collaborating with a broader care team
• Strong organizational and communication skills
• CPR certification and ability to meet clinical onboarding requirements
• Experience in home health, outpatient care, or coordination preferred, not required

• Salary range: $93,000–$99,000 (based on experience)
