We have provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. Responsible for maintaining the delivery of quality patient care by assuring that the Patient Care Plan is followed. The position combines the activities of direct and indirect patient care and provides direct care to pediatric, adolescent, adult and geriatric patients. The Staff Nurse directs CHHAs and CNAs in patient care tasks, performs patient assessments, develops patient care plans, evaluates the plan effectiveness, modifies it as appropriate and documents all patient care provided.
Job Specific Duties:
- Responsible for promoting and maintaining optimal client care. Diagnosing and providing treatment for human responses to actual or potential health problems.
- Makes the initial nursing evaluation visit.
- Re-evaluates the client's nursing needs as required.
- Initiates the plan of care and necessary revisions.
- Provides those services requiring substantial special skills within nurses' scope of Practice Act.
- Assigns, supervises and teaches other clinical personnel, including written para-professional orders and supervision.
- Initiates appropriate preventative and rehabilitative nursing procedures.
- Informs the physician and other personnel of changes in the client's condition and needs.
- Counsels and instructs the client and family in meeting nursing and related needs.
- Prepares clinical and progress notes.
- Participates in in-service training programs.
- Ability to carry out job requirements as instructed and without on-site supervision.
- Assists in coordinating all services provided.
- Refers to appropriate community resources for continuity of client care.
- Participates in required clinical team conferences with other home care staff.
- Performs other duties as assigned in whatever capacity the patients' condition, census, and staffing level requires including rotating work schedule with other RNs, i.e. weekend, on-call and holiday work
Required Education and Experience:
- Registered Nurse with current licensure to practice nursing in the state
- Registered Nurse with minimum (1) year of experience
- Possesses and maintains current CPR certification.
- Bachelor's degree in nursing, preferred
Preferred Education and Experience:
- Must be able to communicate effectively with Residents, Team Members and Visitors and must be able to communicate effectively in English.
- Must be able to function effectively in an organization undergoing change in the model in which patient care is delivered and to support the changes in the model.
- Willingness to participate in the change process by working with the team to problem solve and develop systems to support the highest possible quality care and services to Residents
- Must exhibit tact and patience when dealing with challenging situations.
- Demonstrates professional conduct, service excellence and commitment to our values and mission

Pay: $29.75 - $35.82 per hour
