Job Title: Travel Hospice RN Case Manager

Location: South Dennis, MA

Duration: 13 Weeks (Extension Possible)

Shift: 8:00 AM – 4:00 PM | Day/Evening Shift

Schedule Requirements: Weekday, Weekend, and Holiday Rotation Required

Pay Rate: $2,600 – $3,150 Weekly (Based on Experience)

Job Description

We are seeking an experienced Hospice RN Case Manager for a travel assignment providing compassionate end-of-life and skilled nursing care to patients in home and facility-based settings throughout the Cape Cod region. The ideal candidate will demonstrate strong clinical judgment, exceptional communication skills, and the ability to manage patient care independently while collaborating closely with interdisciplinary healthcare teams.

Responsibilities

• Manage hospice patient caseloads and coordinate individualized care plans
• Perform patient assessments, symptom management, and skilled nursing interventions
• Provide compassionate end-of-life care to patients and emotional support to families
• Communicate effectively with physicians and interdisciplinary care teams regarding changes in patient condition
• Complete admissions, documentation, and clinical records accurately and within required timeframes
• Educate patients, caregivers, and facility staff regarding hospice philosophy, care plans, and symptom management
• Supervise Hospice Aides in accordance with regulatory and organizational standards
• Participate in interdisciplinary case conferences and care planning meetings
• Ensure compliance with hospice care standards, policies, and patient safety guidelines
• Travel throughout assigned service areas to provide patient care in homes and care facilities

Requirements

• Active Massachusetts RN License required
• Minimum 2 years of Hospice RN Case Management experience required
• Hospice Home Care experience required
• Strong patient assessment, communication, and documentation skills
• Ability to work independently in community and home-care environments
• IV Certification preferred
• BSN preferred
• Must successfully pass pre-employment medication examination/testing
