JOB DESCRIPTION & REQUIREMENTS

Registered Nurse – Home Health - Travel - (HH RN) StartDate: ASAP Pay Rate: $2293.03 - $2527.02 The Registered Nurse Case Manager plans and provides individualized nursing care and comprehensive services to home care patients. The Registered Nurse Case Manager assists in clinics and other agency programs as assigned by the Nursing Manager. The Registered Nurse Case Manager is expected to meet established productivity standards (to include weekends/holidays as needed) as well as a rotation of on-call responsibilities. The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned. ? Assesses patient’s health problems and needs and develops plan of care. ? Provides direct skilled nursing care as per physician orders. ? Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action. ? Document medical records and prepare other reports as needed. ? Educates patients and families in self-care and health management. ? Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care. ? Provides case management when appropriate. ? Attending case conference as requested. ? Adheres to policies and procedures. ? Participates in agency quality management and safety activities. ? Must comply with organizational vaccine mandates and maintain required licensure and certification.
