Activities Supervisor (Brockton)

Harbor Health also operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 580 frail elders who continue to live with dignity and independence in the community. Role:
The Elder Services Plan (ESP) Activities Supervisor (AS) is responsible for planning, supervising, and administering a comprehensive therapeutic program for ESP participants in an adult day health setting. The Activity Supervisor is also responsible for preparing activity programs and monitors their effectiveness for participants with varying degrees of memory impairment. Creates initial, semi-annual, and annual recreational therapy assessments based on direct observation and on-going, timely documentation of assessments and interactions with the participants attending the day center.
Responsibilities:
- Implements specialized programming to address the needs of clients with individual and specialized needs.
- Ensures Implementation of daily therapeutic activity programs.
- Recruits, hires, and retrains activity staff ensuring staffing ratios on par with program demands.
- Schedules 1-1 meetings with Direct reports and runs monthly departmental meetings.
- Supervises, instructs, and evaluates activity staff in the performance of their work.
- Ensures group and individual activities are appropriately implemented by designated staff.
- Ensures appropriate interactions with participants and adjustments to plan activities based on participant needs.
- Attends interdisciplinary team and care planning meetings and maintains appropriate records and documentation in collaboration with the Activity staff.
- Assists Site Director/Clinical Director in development of annual operation and capital budgets.
- Orders equipment and supplies.
- Partners with ESP rehab and other departments to integrate Health and Wellness activities.
- Collaborates with external members of the community to provide additional programming for participants.
- Effectively shares analysis of activities and participant status to appropriate team members. Measures and tracks effectiveness of programs. Creatively thinks outside the box to ensure that future programs and activities elicit positive emotions.
- Understands how to delegate duties with staff to ensure center is running effectively and staff are working up to potential.
- Provides re-direction and de-escalation to participants when needed.
- Ensure assessments are completed within 30 days of care plan for participants according to policy and procedures.
- Collaborates with Activity Supervisors at other ESP sites to share strategies and successful activities
Physical Demands:
- Must be able to conduct exercises and engaging group activities for elderly populations.
- Must be able to assist in transferring participants and support participants who are falling.
- Must be able push clients weighing up to 350 lbs. in a wheelchair
- Must be able to lift 15 lbs. of activity boxes and supplies.
- Ability to bend, stoop, and reach and to stand for long periods of time.
- Ability to handle animals as part of Center activities (including, but not limited to puppies, kittens, fowls, and goats).
Requirements:
- BS in health care related fields or BA in human services or related field (4 or more years of related experience may substitute for the Bachelors degree)
- Certified as a Therapeutic Recreational Specialist or Activity Director Certified (ADC) credential from the National Certification Council for Activity Professionals (NCCAP) preferred
- 4 or more years of experience in a social or recreational program providing and coordinating services for a frail or elderly population within the last 5 years, one of which was full-time in a patient activities program in a health care setting
- Highly motivated, creative and energetic
- Must be able to demonstrate teamwork, enthusiasm, and effective communication
- Must be able to communicate sensitively to support meaningful interaction with participants
- Must be able to recognize the signs of distress resulting from confusion and respond by diffusing a person's anxiety and supporting their understanding of the events they experience
- Must be able to demonstrate compassion and vigilance to ensure participant safety
- Working knowledge of physical, psychosocial, and behavioral needs for a frail or elderly population, knowledge of recreational therapy techniques and practices for a frail and elderly population including assessment, care plan development, monitoring and evaluation of needs
- Skills in planning, organizing, and evaluating special activities and programs for frail elders
- Computer literate; must have intermediate skills in Excel, Word, Outlook, social media,
