DEPARTMENT DESCRIPTION:
The CHA Pharmacotherapy Services program has been in place since 2006, growing incrementally from a modest warfarin Anticoagulation Management Service (AMS) at 1 primary care site to its current state with 20 clinical pharmacists and 3 specialized pharmacy technicians providing services in all adult primary care sites, several medical specialty sites, by telephone, and in patients' homes. An estimated 20,000 visits for medication management by our team are expected this year. The specific services provided include anticoagulation, diabetes, hypertension, hyperlipidemia, respiratory, pain, immunizations and behavioral health. The Pharmacotherapy Services team has been successful in improving the quality and safety of care across all service lines, while producing a high level of patient and provider satisfaction. Our team is involved in supporting population health improvement through chronic disease management in a variety of ways.
JOB PURPOSE:
Under the general supervision of the Supervisor, Pharmacotherapy Services, and within established organizational and departmental policies and procedures, the Specialist, Pharmacotherapy Operations is responsible for providing high level administrative support of the Pharmacotherapy Service practice within defined practice areas at the Cambridge Health Alliance (CHA). Primary, essential job functions include serving as a patient liaison with knowledge of pharmacology, drug therapy, drug/food interactions and current drug treatments in all areas of responsibility, recognizing clinically significant issues, outcomes, and prioritizing the communication of such expeditiously, accurately, and effectively for delivery of excellent service in accordance with established standards of care; Providing high-quality laboratory testing, data management, processing device applications, project coordination, maintenance of patient care information, and maintenance of pharmacists’ schedule templates. A commitment to patient safety, customer satisfaction, teamwork, innovation, and performance improvement is required.
QUALIFICATIONS/REQUIREMENTS:
Education/Training:
High school diploma or G.E.D. certificate is required.
Associates or Bachelors degree preferred.
Licensure: N/A.
Registration: Must maintain a current pharmacy technician registration in good standing from the Massachusetts Board of Registration in Pharmacy.
Certification:
Must maintain a current certification in good standing by the Pharmacy Technician Certification Board (PTCB).
Must maintain phlebotomy certification, initially provided upon training by CHA laboratory.
Work Experience: Minimum of five (5) years experience as a pharmacy technician or three (3) years with a college degree is required. In addition, a demonstrated knowledge of pharmacology, drug therapy and current drug treatments in all areas of responsibility is preferred.
