Registered Nurse – Behavioral Health (PACT Team)
Location: Worcester, MA
Shift: 40 hrs/week (Day shift – mix of 8s & 10s)
Start Date: 05/04/2026
Duration: 13 weeks
Requirements:
- 1.5+ years RN experience
- Active MA RN license
- Flexible schedule (40 hrs/week guaranteed)
- Willing to travel/transport patients if needed (personal vehicle may be required)
- Comfortable in community-based/outpatient behavioral health
Certifications:
- BLS (Required)
Skills:
- Behavioral health nursing (mood, psychotic, substance use, trauma disorders)
- Suicide risk assessment & crisis care
- Medication administration, refills & prior auths
- Nursing assessments (medical, emotional, social)
- Seclusion/physical hold protocols
- Patient education & therapeutic communication
- Wellness education (nutrition, smoking cessation, stress management)
- Care coordination (psych + primary care)
- EMR documentation
Responsibilities:
- Medication refills, education, and prior authorizations
- Complete nursing assessments (diabetes, HTN, TB screening, etc.)
- Identify patient needs and coordinate follow-up care/referrals
- Develop and update individualized care plans
- Provide wellness education and group support
- Coordinate care between psychiatry, primary care, and support systems
- Document all care in EMR daily
- Provide community-based nursing support as needed
