Patient Care Manager – Norwood
Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.
Key Responsibilities
- Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
- Coordinate and monitor patient care delivery
- Supervise clinical staff to ensure consistent, appropriate service
- Maintain clear communication with patients, caregivers, referral sources, and field and office staff
- Drive quality outcomes by meeting department goals and supporting improvement initiatives
- Model professionalism and organizational values in daily work
- Uphold compliance through required training, accurate reporting, and cooperation with audits
- Promote a safe work environment by following established safety protocols
- Support staff growth through coaching and continuing education
Patient Care Manager Qualifications
- Registered nurse with current licensure to practice nursing in the practicing state.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
