Home Care Case Assistant $21.75/hour
Position Summary : This position is responsible for receiving calls about Home Care clients and their services, providing follow up to calls, taking messages, routing calls, completing initial skills training for consumers in self-directed programs, and performing a variety of office support tasks.
Principal Accountabilities:
Ensures that callers are greeted in a friendly manner and on a timely basis:
- Identifies priority calls.
- Provides follow up to clients and vendors as appropriate.
- Routes calls.
- Takes and tracks messages.
Completes initial skills training s for consumers in Consumer Directed Care (CDC) Program.
- Completes home visits with consumers in the community.
- Assists with completion of paperwork to enroll consumers in CDC Program.
- Trains consumers and/or surrogates on all aspects of the CDC Program.
- Submits and tracks all employment paperwork related to CDC to the Fiscal Intermediary.
Supports home care staff by performing a variety of tasks:
- Mails notices and paperwork.
- Contacts providers and arranges service for clients.
- Contacts clients to gather and provide information.
- Runs reports for quality control.
- Prepares documents and photocopies.
- Completes filing, including managing terminated records.
- Assists with community activities, when needed.
- Participates in team meetings.
Qualifications:
Education: High school diploma or equivalent.
Experience: One year of telephone answering and general office experience.
Special Skills/Knowledge:
- Pleasant telephone manner and basic telephone etiquette.
- Ability to work with consumers in a community setting.
- Excellent oral and written communications skills,
- ability to effectively relate to a wide variety of personalities,
- basic computer skills,
- organizational and time management skills,
- ability to multi-task and work in a team-oriented, fast-paced, high volume work environment.
- Ability to speak Portuguese or Spanish helpful.
- Driver’s license and reliable transportation required.
