- Shift: Day Shift (7:30 AM – 4:00 PM)

Job Summary
We are seeking an experienced Registered Nurse – Case Manager to join a dynamic healthcare team at Anna Jaques Hospital. Case Managers play a critical role in coordinating patient care, ensuring appropriate utilization of services, and facilitating safe and effective discharge planning.
Key Responsibilities
- Coordinate care for patients, families, and caregivers
- Perform concurrent and retrospective utilization reviews
- Ensure compliance with regulatory and clinical guidelines
- Develop and implement safe discharge plans
- Collaborate with interdisciplinary teams and external agencies
- Prevent denials through proper documentation and review processes
- Use established criteria sets to support medical necessity
Requirements
- Active Massachusetts RN License (Required)
- BLS Certification (AHA) (Required)
- Graduate of an accredited nursing program ( BSN preferred )
- Epic EMR experience required
- Strong background in case management and care coordination
- Must be separated from Beth Israel Lahey Health for at least 6 months and in good standing
