Responsibilities
TLC Nursing Associates, Inc. is looking for an experienced Travel RN – Care Manager to coordinate patient care across multiple healthcare settings. The Care Manager will assess, plan, and implement care strategies to ensure quality patient outcomes while optimizing resource utilization. The ideal candidate will have strong case management, discharge planning, and patient advocacy skills to provide comprehensive care for patients with complex health needs.
Qualifications
- Develop and implement individualized care plans for patients
- Coordinate patient care across hospitals, rehabilitation centers, home care, and outpatient facilities
- Collaborate with physicians, social workers, and multidisciplinary teams to ensure effective treatment plans
- Assess patient needs, risks, and barriers to care and provide solutions
- Educate patients and families on treatment plans, medication management, and healthcare resources
- Monitor patient progress and adjust care plans as needed
- Ensure compliance with healthcare regulations, insurance requirements, and hospital policies
- Facilitate smooth transitions of care from hospital to home or other care settings
