Registered Nurse Clinical Care Manager

Contract Details:

• Pay: Up to $55 per hour
• Employment Type: Full-time

Key Responsibilities:

• Conduct comprehensive clinical assessments to evaluate patient needs and develop tailored care plans
• Coordinate services across multidisciplinary teams to ensure seamless, continuous care delivery
• Educate members and their families on treatment plans, health management, and adherence strategies
• Monitor patient progress, adjusting care strategies as needed to meet changing health requirements
• Maintain compliance with case management principles, regulatory standards, and NCQA requirements
• Document all interactions and care plans accurately in healthcare databases, ensuring organizational and regulatory standards are upheld

Requirements:

• Minimum of 2 years of RN experience in inpatient, outpatient, or other healthcare settings
• Active RN licensure in Massachusetts
• Proficiency in electronic health record documentation systems
• Excellent verbal communication skills and comfort working via phone
• Current residency in Massachusetts

Nice-to-Have Skills:

• Prior case management or care management experience
• Experience working with diverse populations and individuals with complex needs
• Knowledge of social determinants of health and strategies to address care disparities

Preferred Education and Experience:

• Bachelor's degree in Nursing (BSN) preferred
• Certifications such as CCM (Case Management Certification), CDE (Certified Diabetes Educator), or Wellcoach are advantageous
