Contract Type: Travel Contract

Shift Type: 8-hour day shifts, 40 hours guaranteed per week. No weekends, holiday coverage if required. No on-call.

Roles & Responsibilities

• Provide and document skilled nursing care to patients in their homes
• Promote, maintain, and restore health through evidence-based interventions
• Collaborate with healthcare providers and community resources to coordinate care
• Evaluate patient and family responses to care and adjust plans as needed
• Educate patients and families on disease management and prevention
• Independently manage your caseload with minimal supervision

Qualifications

• Active Massachusetts LPN license (must upload with profile)
• Current AHA BLS or American Red Cross CPR certification (must upload)
• One year of acute nursing care experience (hospital preferred)
• Home health experience required
• Certified Home Health with OASIS proficiency highly preferred
• Infusion therapy experience preferred
• Valid driver's license and auto insurance required
• Reliable personal vehicle for home visits (firm requirement)
• Willingness to commute up to 45 minutes from the office in Pittsfield

Knowledge & Skills

• Strong critical thinking and problem-solving abilities
• Excellent communication and interpersonal skills
• Time management and organizational skills for self-scheduling
• Knowledge of community health principles and home care reimbursement
• Ability to work independently and exercise independent judgment
• Comfortable driving in inclement weather and accepting on-call responsibilities
• Proficiency in OASIS documentation and infusion therapy
