Care Management Nurse – Remote

We are looking for a Care Management Nurse to help patients with complex health needs achieve better outcomes. This fully remote role is open to Massachusetts-based RNs who want to apply their clinical expertise in a supportive, collaborative environment while enjoying work-life flexibility.

Key Responsibilities:

  • Complete clinical assessments and design individualized care plans
  • Coordinate patient care with physicians, specialists, and community resources
  • Educate patients on managing chronic conditions and achieving health goals

Requirements (Must-Have):

  • Active RN license in Massachusetts
  • At least 2 years of clinical nursing experience (inpatient, outpatient, or similar)
  • Strong assessment, problem-solving, and communication skills

Nice-to-Have Experience:

  • Case Management certification (CCM) or related credential
  • Training in diabetes education or health coaching (e.g., Wellcoach)
  • Experience addressing social determinants of health and reducing care disparities
