Forensic Mental Health Clinician
The Master’s Level Clinician provides community-based services within the Program of Assertive Community Treatment (PACT) Forensic model of care which includes services including therapy, medication administration, assistance with ADL’s, and care coordination.
What You'll Do- Provides comprehensive Intake Evaluations with clinical formulations and recommendations based on sound clinical principles and judgement.
- Provides individual, family, couples, and/or group treatment based on sound clinical theories and principles.
- Includes all pertinent family members, community, and residential providers in treatment, as appropriate.
- Maintains direct services productivity as established by the Program.
- Assures assigned individuals served make positive change on the outcome measure used by the PACT Model.
- Provides comprehensive risk/crisis evaluations and takes appropriate actions to keep individuals served and others safe.
- Maintains flexibility in providing services in settings such as individuals served homes, hospitals, and other community settings.
- Maintains professional licensure and completes the required amount of clinical education hours.
- Assists and monitors activities of daily living.
- Encourages independence, competency, and decision-making.
- Initiates, supports, and ensures quality of life through empowerment, safety awareness, respect, and dignity.
- Provides outreach services to individuals served on a frequent basis as required by supervisor.
- Acts as primary implementer of strategies to meet Treatment Plan goals.
- Completes risk plans and crisis plans as required by supervisor.
- Completes progress notes for all individuals by the end of each shift.
- Ensures Smartsheet is updated each shift and that change of shift report occurs each shift with oncoming staff.
- Completes program specific forms and required time-sensitive documents such as Incident Reports and Medication Occurrence Reports in a timely manner.
- Completes health and medication-related documentation as individually indicated and in accordance with MAP regulations.
- Provides on-going information, periodic reviews, and documentation to support treatment and Treatment Plan goals and objectives.
- Completes initial assessments, treatment plans, treatment plan reviews, financial management plans, and assessments to assigned caseload as required by supervisor.
- Serves as primary case manager for assigned caseload as required by supervisor.
- Reviews and becomes familiar with PACT orientation manual and Policy & Procedure manual.
- Works weekends, second shifts, and holidays as needed by supervisor.
- Attains competence in motivational interviewing and utilizes this approach when working with ambivalence.
- Provides safe and timely transportation in own vehicle, as needed for medical appointments, recreational activities, shopping, errands, etc.
- Performs cleaning and maintenance at residence and during client incidents in regard to hazardous materials and bloodborne pathogens to be in compliance with OSHA guidelines.
- Attends in-services, meetings, and trainings as assigned and scheduled, keeping mandatory trainings up to date.
- Completes individually assigned program-specific responsibilities
- Recognizes and reports program-specific safety and maintenance concerns.

- Hours are Mon 8a-4p, Tues 12p-8p, Wed 8a-4p, Thurs 8a-4p, Fri 8a-4p
- Master’s Degree in Social Work, Psychology, or Mental Health Counseling required.
- Licensure Preferred (LICSW, LCSW, LMHC).
- License eligible encouraged to apply.
- Minimum of six months experience in human services preferred.

- Excellent verbal and written communication skills.
- Demonstrates willingness and ability to treat challenging and diverse client populations, including a forensic population.
- Demonstrates cultural competence in supporting clients with a variety of different cultural backgrounds.
- Demonstrates working knowledge of community resources, and how to help clients access them.
- Demonstrates willingness to work with individuals who are forensically involved within the Sequential Intercept model.
- Valid driver's license and a reliable vehicle available for all working hours to transport individuals served required.
- Must obtain and maintain MAP certification within introductory period of employment (6 months).
