Qualifications
- Education: Active Registered Nurse (RN) license .
- Experience: Minimum 2+ years in case management, discharge planning, or care coordination .

- Certifications: BLS required; Case Management Certification (CCM) preferred .
- Travel Readiness: Willingness to accept travel assignments .
