Responsibilities
- Conduct comprehensive speech-language pathology evaluations for residents and create individualized treatment plans.
- Provide therapy addressing speech, language, cognitive-communication, and swallowing disorders; implement targeted therapeutic activities.
- Document progress notes, treatment plans, and discharge summaries in the resident’s chart; ensure regulatory and facility compliance.
- Collaborate with an interdisciplinary team including nursing, rehabilitation professionals, and social services to optimize resident outcomes.
- Educate residents, families, and caregivers on communication strategies and swallowing safety; participate in discharge planning as appropriate.

Requirements
- Ability to work in a skilled nursing facility setting and within a resident-centered care model.
- Strong communication, documentation, and teamwork skills.
- Demonstrated clinical skills in evaluation and treatment of speech, language, and swallowing disorders.
- Willingness to complete assignment as a travel clinician and comply with facility policies and timelines.
