Physical Therapist - Travel Contract
Looking for a change of scenery and a chance to help others. Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services. Utilize timely electronic documentation and interdisciplinary communications in accordance with agency policies and standards of practice for home health.
Minimum Requirements
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Massachusetts Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 1 year of certified home health experience. Experience must be within a certified home health agency; hospice, private care, or standalone case management roles do not meet this requirement.
- Minimum 1 year of case management experience.
- Ability to travel to patient cases as assigned. Weekend rotations occur approximately once every six weeks.
Preferred Qualifications
- Experience with electronic documentation systems in home health settings.
- Previous interdisciplinary collaboration in home care or community-based practice.
Assignment Details
- Shift hours: 08:00–16:30.
- Shift days: Days.
- Weekend rotation: approximately once every six weeks.
- Ability to travel to cases across the region, including Lynn, Nahant, and Saugus.
