Cooperative for Human Services Inc. (CHS) is seeking a passionate and experienced Individual Supports Program Manager to join our dynamic leadership team. In this vital role, you’ll guide our Individual Supports Coordinators and ensure that each person we serve receives personalized, empowering, and high-quality support services. - Champion person-centered supports by ensuring that services reflect the individual’s unique needs, preferences, and goals.
- Lead and inspire a team of Individual Supports Coordinators, providing training, guidance, and performance management.
- Coordinate and monitor ISPs , ensuring goals are implemented, documented, and regularly reviewed in collaboration with each person’s support team.
- Act as an advocate and point of contact for individuals, families, guardians, service providers, DDS, and healthcare professionals.
- Respond to emergencies and provide on-call support as needed.
- Ensure program quality and compliance , including documentation, audits, and alignment with DDS and CHS standards.
- Promote wellness and safety , including oversight of health, nutrition, medication administration, and safe environments.
- Foster community connections and expand access to resources, activities, and opportunities for independence.
- Provide transportation for individuals and support their participation in appointments or community events.

- Bachelor’s degree in a related field (required).
- Minimum of 3 years' experience working with people with developmental disabilities.
- At least 1 year of supervisory experience.
- Strong understanding of DDS regulations and person-centered planning practices.
- Excellent written and verbal communication skills.
- High emotional intelligence, sound judgment, and leadership presence.
- Valid driver’s license and reliable vehicle.
- Must be able to lift 30 lbs and meet physical requirements of the position.
