Job Description: Clinical Director – Home Health Agency (Start-Up Environment)
Position Overview: We are seeking an innovative and dedicated, and experienced Nurse Manager to lead our nursing team in a home health agency with lots of room to make an impact and foster growth. This role is crucial in establishing and shaping our nursing services, ensuring high-quality care delivery, and fostering a positive team culture. The Nurse Manager will work collaboratively with the leadership team to develop policies, procedures, and practices that align with our agency's mission and values.
Key Responsibilities:
Leadership and Management:
- Recruit, train, and supervise nursing staff, ensuring a cohesive and effective team.
- Establish and promote a positive work environment that supports staff development and retention.
Care Coordination:
- Oversee the development and implementation of individualized patient care plans.
- Ensure that care is delivered in accordance with best practices and regulatory standards.
Quality Assurance:
- Implement quality improvement initiatives to monitor and enhance patient outcomes.
- Conduct regular audits of patient records and care processes to ensure compliance.
Regulatory Compliance:
- Develop and maintain policies and procedures that comply with state and federal regulations.
- Prepare for and participate in regulatory visits and inspections.
Patient and Family Engagement:
- Serve as a primary point of contact for patients and their families, addressing concerns and providing education about care plans.
- Foster strong relationships with patients to enhance satisfaction and trust.
Budgeting and Resource Management:
- Assist in developing and managing the nursing department budget, ensuring efficient use of resources.
- Monitor staffing levels and schedules to meet patient needs while managing costs.
Emergency Preparedness:
- Create and implement emergency response protocols to ensure patient safety.
- Train staff on emergency procedures and best practices.
Qualifications:
Education:
- Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred.
Licensure:
- Current state RN license in good standing.
Experience:
- Minimum of 3-5 years of nursing experience, with at least 1-2 years in a management or supervisory role, preferably in home health or a start-up environment.
Skills:
- Strong leadership and organizational skills.
- Excellent communication and interpersonal abilities.
- Familiarity with electronic health records (EHR) and home health technology.
- Strong Oasis completion skills in accordance with Medicare regulations is a must.
Certifications:
- Certification in Home Health Nursing (CHHN) or related specialty is a plus.
Work Environment: This position involves a blend of office work and travel to patients’ homes. Flexibility in scheduling may be necessary as the agency grows and evolves.
