Full-Time Telehealth RN

Location: Boston, MA (Boylston Street)

Schedule: Monday-Friday | 9:00 AM-5:00 PM

Assignment Type: 13-week contract to start

Pay Rate: $55/hour

Position Overview

Amergis Healthcare Staffing is partnering with a well-respected Adult Day Health organization committed to helping older adults and individuals with disabilities remain safe, independent, and supported in their homes. Through individualized care planning, clinical oversight, caregiver education, and case management, this organization delivers high-quality, community-focused care throughout Boston.

As a Telehealth RN, you will play a key role in conducting clinical assessments, monitoring ongoing needs, and coordinating services to ensure continuity of care. You'll collaborate closely with an interdisciplinary team, caregivers, and healthcare providers to develop proactive, patient-centered care plans that promote safety, dignity, and quality of life.

This role is ideal for an RN seeking autonomy, relationship-based care, and meaningful community impact-without inpatient or shift-based demands.

Key Responsibilities

• Complete telehealth intakes, annual assessments, and admissions

• Complete all required documentation, including client assessments and multidisciplinary plans of care

• Communicate professionally with clients' PCPs and other healthcare providers as needed

• Review admission materials with clients and caregivers to ensure understanding

• Educate caregivers on required communication tools or electronic visit verification systems

• Conduct regular in-home visits, including caregiver education and skills training

• Assess changes in health status, environment, and psychosocial needs

• Complete semi-annual visits, including care plan updates, goal reviews, medication reconciliation, and caregiver tool reviews

• Participate in multidisciplinary team meetings

• Perform other duties within scope of practice as assigned

Qualifications

• Active Massachusetts RN license in good standing (required)

• 2+ years of nursing experience preferred

• Strong clinical assessment and critical-thinking skills

• Ability to work independently and manage time effectively

• Proficiency with Microsoft Office and EMR/documentation systems

• Comfortable using telehealth tools (video, chat, etc.)

• Prior case management or home health experience preferred, not required
