Position: Adult Family Care Case Manager
Location: Wakefield, MA
Pay rate: $49,920 exempt annual salary
Hours: Full-time, 40 hours per week – Hours worked at the Communitas office in Wakefield and in the community attending home visits (Some work from home hours can be accommodated upon completion of 90-day probationary period) Benefits of Working for us!
- Tiered pay increases based on years of service, pending performance evaluations
- 4 weeks’ vacation, 1 week sick time, and 12 holidays for new full-time employees

- Tuition reimbursement eligibility after 90 days of employment
- Conducting the initial and ongoing assessments of the AFC qualified setting.
- Obtaining a social history and conducting a psychosocial assessment and evaluation of the participant.
- Assisting in selection, training, evaluation, and supervising of AFC caregivers in conjunction with the Director.
- Conducting on-site visits with each participant and the caregiver in the qualified setting monthly or bi-monthly. Completing progress notes, ensuring the documentation is completed in a timely manner. Conducting periodic review of AFC care logs.
- Assisting the participant in obtaining information and accessing other healthcare and community services.
- Participating in ISP and team meetings as needed, attends other meetings as required.
- Bachelor’s Degree in Social Work, Education, Human Services, or a related field is required.
- At least two years of experience in the care of elders or those with disabilities.
- A passion and dedication to supporting our people is a must
- Valid Driver’s License and acceptable driving record
