Baystate Health Home Health Aide

This position provides support and assistance with personal care. Assists patient in functioning in their home setting and encourages independence when possible. Maintains safe, healthy environment for patients by completing assigned tasks on the Home Care Aide Plan of Care.

Schedule:

• Full-time day shift
• 8:00am 4:30pm with some flexibility
• Weekend & holiday rotation
• On the road Hampden/Hampshire county (mileage reimbursement)

HHA requirements:

• Home Health Aide or CNA Certification
• Valid Drivers License
• HiSet/GED or High School Diploma

Position responsibilities:

• The Home Health Aide provides emotional and physical support to terminally ill individuals and their caregivers.
• The Home Health Aide assists with all personal care duties in a caring and professional manner, encouraging patient independence within the patients functional limitations.
• The Home Health Aide assists the caregiver in teaching self care to the patient to maintain and promote the independence of individuals in a safe and healthy environment.
• The Home Health Aide performs duties indicated in the patient care plan as outlined by clinical staff and demonstrates good judgment and skill and takes appropriate initiative to insure that patient care is of optimum quality.

Education:

GED or HiSET (Required)

Certifications:

Driver License Other
