Job Title: Social Worker – LICSW

Location: Hyannis, MA

Duration: 26 Weeks

Schedule & Shift

• Shift: Day Shift (08:00 AM – 04:00 PM)
• Schedule: 5x8 Hours | 40 Hours/Week

Position Overview

The LICSW Social Worker provides comprehensive psychosocial assessments, clinical support, discharge planning, and care coordination in an acute care hospital setting. This role focuses on supporting patients and families through medical, emotional, and social challenges while ensuring safe and effective transitions of care.

Key Responsibilities

• Conduct psychosocial assessments and provide clinical support
• Perform discharge planning and coordinate care transitions
• Provide patient and family counseling and advocacy
• Connect patients with community resources and services
• Maintain accurate documentation and ensure compliance

Required Qualifications

• Master’s Degree in Social Work (MSW) – Required
• Active LICSW State License – Required
• Strong clinical assessment and crisis intervention skills
• Ability to manage multiple cases in a fast-paced setting
• Excellent communication and organizational skills
• Experience with EHR systems

Certifications & Requirements

• LICSW License (Required)
• CPI / MAB Certification within 1 month of hire (Required)
• English proficiency (read, write, communicate) required
• Strong adherence to professional ethics and hospital policies
