Nurse Case Manager - Up to $55/hr

We are urgently hiring a Nurse Case Manager (RN) to provide patient-centered care coordination for individuals with complex medical needs. In this role, you’ll use your nursing expertise to guide members through personalized care plans, ensuring they receive the right resources and support to achieve their health goals.

Key Responsibilities:

• Perform thorough clinical assessments and develop tailored care plans
• Coordinate across providers and multidisciplinary teams to ensure continuity of care
• Educate and engage patients and families to support treatment adherence and health goals
• Maintain accurate documentation in compliance with regulatory and organizational standards

Requirements:

• Active RN licensure in Massachusetts
• Minimum 2 years of RN experience (inpatient, outpatient, or varying levels of care)
• Strong clinical assessment and problem-solving skills
• Must be currently living in Massachusetts

Preferred Skills & Experience:

• Case management experience
• Certifications such as CCM, Certified Diabetes Educator, or Wellcoach a plus
• Proficiency with healthcare IT systems and clinical documentation
• Experience addressing social determinants of health and working with diverse populations
