Bilingual Geriatric Support Services Coordinator (Spanish)

Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid (in-person & remote) work schedule (once training is complete.) No nights or weekends.

Compensation: Starting annual salary is $ 53,924.80 which includes an additional 6% bilingual designation for Spanish fluency. Job Activities

Job activities of the position are below. Additional duties may be assigned as required.

  • Conduct initial and on-going in-home assessment of consumers' health and functional status.
  • Participate in development of consumers' care plans, including determining appropriateness and need for in-home/community-based services.
  • Participate as part of consumer's primary care team including, attending team meetings, trainings, and joint home visits.
  • Arrange, coordinate, and oversee in-home and community-based services for consumers.
  • Monitor consumer's health status, in conjunction with care team, to assess for effectiveness of current services, need for additional services, and/or need for long-term placement.
  • Assist consumers with MassHealth redeterminations and other benefits paperwork, as requested and appropriate.
  • Maintain consumer records in accordance with standards.
  • Compile and submit required reports.

Qualifications

  • Fluent in English and Spanish
  • A Bachelor's degree in Social Work or a related field preferred.
  • Social Work license preferred.
  • Knowledge of and experience working with older adults in a community-based setting preferred.
  • Strong interviewing and assessment skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills in English and Spanish.
  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
  • Private transportation and valid license
