Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
• Active MA Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience preferred. Candidates with Hospital or SNF experience will be considered.
• Experience using electronic medical records. Familiarity with Homecare Homebase preferred.
• Ability to meet productivity target of 6 points/day (30 points/week).
• Valid driver‘s license and reliable transportation for patient visits within a manageable territory.
• May consider a PT/COTA pair; COTA applicants may be considered when submitted with a PT.

• Shift name: Days.
• Schedule options: 5x8 (Monday–Friday) or 4x10s.
• Primary coverage area includes Northbridge, Upton, Uxbridge, Mendon, Millville, Blackstone, and Bellingham. Territory is manageable with street parking and driveways available.
• EMR: Homecare Homebase.
• On call: None.
