Job Title: Care Management Nurse Profession: RN Specialty: Care Management Duration: 13 weeks Shift: 8 hours Hours per Shift: 8 Experience: Minimum of 3 years required; Preferred 5+ years License: BSN required Certifications: CCM or related certifications preferred Must-Have: - Extensive background and knowledge of current care patterns of practice and acute inpatient care. - Proficiency in methods for assessing physical and mental impairment. - Understanding of the characteristics of illness and expected treatment. - Knowledge of benefits, plan contracts, eligibility issues, and requirements. Description: This role involves utilizing a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet patients' health needs. Good communication skills are essential in promoting quality and cost-effective outcomes for 18-22 patients. The care manager will meet patient needs, manage length of stay, and promote efficient utilization of resources.
