Job Title: Licensed Practical Nurse (LPN/LVN) – Long-Term Care

Location: Falmouth, MA

Job Type: Contract

Duration: 13 Weeks

Schedule:

• 40 hours per week
• Evening Shift: 3:00 PM – 11:30 PM
• Every other weekend required
• Rotating holidays

Compensation

• Travel Rate: $48 – $51/hour ($1,920 – $2,040/week) – based on experience
• Local Rate: $40 – $45/hour ($1,600 – $1,800/week) – based on experience

Position Overview

A well-established healthcare facility is seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide high-quality nursing care in a long-term care (LTC) setting. The LPN will oversee nursing assistants and ensure compliance with all applicable regulations while delivering compassionate, resident-centered care.

Responsibilities

• Supervise and direct daily activities of nursing assistants in accordance with facility policies and regulations
• Coordinate shift activities, including care plans and resident services
• Complete and maintain accurate documentation for admissions, transfers, and discharges
• Communicate with physicians regarding treatment plans, orders, and progress notes
• Receive and transcribe physician orders into resident charts and care plans
• Maintain detailed and accurate nursing notes reflecting patient care and outcomes
• Ensure compliance with federal, state, and local healthcare regulations
• Promote a safe and high-quality care environment for all residents

Required Qualifications

• Graduate of an accredited nursing program or approved LPN program
• Minimum 1 year of experience in a long-term care (LTC) setting
• Active, unrestricted LPN license in the state of Massachusetts
• BLS / CPR / AED Certification required
• Strong verbal and written communication skills in English
• Ability to provide age-appropriate care based on resident needs
