What You Bring to the Table:

• Valid RN licensure in Massachusetts with active registration
• Minimum of 2 years’ clinical experience in inpatient, outpatient, or similar settings
• Strong clinical assessment and problem-solving skills, with the ability to interpret complex medical information
• Excellent communication skills, both verbal and written, with a knack for patient engagement and education
• Ability to work independently and collaboratively in a fast-paced, self-directed environment
• Must be currently living in Massachusetts

Nice to Have Skills:

• Case Management certification (CCM) or similar credentials
• Experience with diabetes education or related certifications such as Wellcoach
• Familiarity with social determinants of health and care disparity interventions
• Previous experience in managing medication plans and coordinating multidisciplinary teams
