Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: Current licensure in the state of Massachusetts Certifications: None specified Must-Have: 1. 1 year Certified Home Care experience 2. 1 year Case Management Experience 3. Reliable, independent means of transportation to make home visits 4. Willingness to work weekends, holidays, and on-call as needed Description: Providing clinical services to patients from birth to over 100 years in their homes. Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Performing admissions processes and case management responsibilities in accordance with policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications. Weekends rotations are once every 6 weeks. About Cynet Health
