Job responsibilities:
- Provide therapeutic interventions and track patient progress toward measurable goals.
- Collaborate with the care team to coordinate therapy plans and discharge recommendations.
- Work 40 hours per week with 8-hour shifts.
- Support safe mobility and use standardized safety practices during sessions.
- Communicate functional changes and barriers to the care team to support care planning.
