Better Care Home Health, Inc. is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Part-Time Home Health and Hospice Nurse. In this role, you will provide skilled nursing care to patients in their homes, ensuring comfort, dignity, and quality of lifeespecially during end-of-life care. This position requires strong clinical skills, excellent communication, and a deep understanding of hospice philosophy and home health protocols.. This position is FULLTIME (40 hours), and is benefit-eligible.
Home Health & Hospice Nurse (RN) duties and responsibilities:
Deliver skilled nursing care and assessments to patients in accordance with the plan of care.
Provide end-of-life care in alignment with hospice principles, ensuring patient comfort and family support.
Educate patients, caregivers, and families on disease processes, medications, and care techniques.
Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides.
Accurately document clinical findings, interventions, and patient responses in the electronic medical record (EMR).
Monitor and report changes in patient condition to the supervising physician or hospice medical director.
Participate in case conferences and team meetings as needed.
Maintain compliance with state and federal regulations, as well as agency policies and procedures.
Requirements:
Internet-capable Computer, Smart Phone and/or Tablet (iPad/Android) for accessing and working within the Electronic Medical Record (EMR).
All applicants will be required to submit a background check authorization with the Massachusetts Criminal Offender Information Registry (CORI).

A valid, unrestricted MA drivers license and reliable transportation.
Valid, unrestricted license as an RN in the State of Massachusetts.
Minimum of 2 years of hospice nursing experience required.
Home Health Experience Preferred.
Part-time hours, flexible scheduling based on patient needs and agency coverage.
