Community Health Worker - Behavioral Health
Salary for this position: $19.00- $22.00/Hour

We are currently hiring a Community Health Worker – Behavioral Health based in Worcester. As part of the care management team, the Community Health Worker (CHW) works as a part of the health care team to provide comprehensive, coordinated CHW interventions in accordance with the Massachusetts Board of Certification of Community Health Workers core competencies as part of a patient centered health care home for Health Center patients.

Essential Functions:
Provides health system education and advocacy to help patients navigate care and access needed resources.
Supports patients facing mental health, emotional, or substance-use challenges with culturally appropriate connections to services.
Conducts SDOH assessments and creates intervention plans based on identified needs.
Assists patients with housing, transportation, education, employment, child care, food security, and utilities.
Follows up with referred patients to address social barriers affecting engagement in care and treatment adherence.
Participates in population-based projects and meets grant-related requirements.
Performs other duties as assigned.
Required Qualifications:
- Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.
- Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
- Excellent written and oral communication skills.
- Successful completion of the CHW core competency training within 2 years of employment.
