We are seeking a licensed Pharmacist to join our team! As a Pharmacist in our long-term care pharmacy, you will play a vital role in ensuring the health and well-being of our patients. You will be responsible for dispensing medications, monitoring patient profiles, and providing clinical expertise..
Responsibilities:
- Dispense medications and supplies to patients in long-term care facilities
- Review and monitor patient profiles for potential medication interactions, allergies, and contraindications
- Provide clinical consultations to healthcare professionals, patients, and caregivers
- Participate in medication regimen reviews and make recommendations for optimization
- Collaborate with our pharmacy team to ensure accurate and timely delivery of medications
- Maintain accurate records and reports as required by state and federal regulations
- Stay up-to-date with industry developments, new medications, and changing regulations
- Participate in quality improvement initiatives to enhance patient care and safety
Requirements:
- Current licensure as a Pharmacist in the state of Massachusetts
- Experience in long-term care pharmacy or geriatric care preferred
- Strong clinical knowledge and analytical skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Proficiency in pharmacy software and electronic health records
