Position Overview : The Outreach Specialist for NRLP will initiate outreach efforts to referred individuals, introducing the program and enrolling those interested in participating.
Position Responsibilities: Initiates outreach efforts to potential enrollees. Provides information about the LTSS option to enrollee, including a description of the role of the LTSS Care Coordinator and the enrollment process, using skills such as motivational interviewing to engage enrollees. Ensures quality communication and dialogue with enrollees, internal staff, and external partners. Provides general outreach support.
Qualifications:
- Bachelor's Degree in human services, social work, or related field preferred. Minimum of an Associate's degree required
- Five years' experience preferred working with individuals of all ages and living with various disabilities using a consumer directed approach or demonstrated experience of at least two years
- Must be organized, able to multitask, and work well within a team in a fast-paced environment and be self-directed
- Proficiency with Microsoft Word, Excel, Outlook, and PowerPoint
- Strong desire to help others and advocate for those needing services and assistance
- Excellent communication skills with the ability to establish positive and professional relationships with team members, providers, consumers, and their support system
- Bilingual Spanish is highly preferred
- Valid Driver License and Reliable transportation with the ability to conduct home visits throughout the service area
Hours: Full Time - 40 hours Monday through Friday
