Position Summary:
The Director of Home and Community-Based Services is responsible for the effective operation, performance, and continuous improvement of SCES’s Home and Community-Based Services portfolio, including State Home Care Programs, Managed Care Programs, Community Nursing, Housing Supports, Transitions of Care, and other assigned programs and projects.
Reporting to the Chief Operating Officer, the Director provides operational leadership and accountability for HCBS service delivery, ensuring programs achieve established quality, compliance, financial, workforce, and performance expectations. This role is responsible for translating organizational priorities into effective day-to-day execution by strengthening operational practices, supporting managers, monitoring performance, resolving barriers, and ensuring teams have the clarity, resources, and support needed to deliver high-quality, person-centered services.
As a member of the SCES Leadership Team, the Director contributes to organizational planning and supports execution of strategic priorities by developing operational plans, monitoring progress, addressing challenges, and ensuring consistent follow-through. The successful candidate creates a culture of accountability, collaboration, operational excellence, regulatory readiness, and person-centered service.
Key Responsibilities:
1. Programmatic Operational Leadership
· Provide day-to-day operational leadership for SCES’s HCBS programs, ensuring effective service delivery, regulatory compliance, financial stewardship, and achievement of performance expectations.
· Translate organizational priorities into clear operational goals, workflows, timelines, and accountability measures.
2. Execution of Organizational Priorities & Continuous Improvement
· Support implementation of SCES strategic priorities by translating organizational goals into practical operational plans and measurable actions.
· Develop and monitor HCBS improvement plans, ensuring initiatives have clear ownership, timelines, and expected outcomes.
· Partner with Directors and department leaders to coordinate implementation efforts and resolve barriers to successful execution. Provide day-to-day operational leadership for SCES’s HCBS programs, ensuring effective service delivery, regulatory compliance, financial stewardship, and achievement of performance expectations.
3. Regulatory Compliance & Quality Management
· Ensure departmental compliance with all applicable federal, state, contractual, payer, and accreditation requirements.
· Develop and maintain operational systems that proactively monitor compliance, quality, and program performance, identifying risks before deficiencies occur.
· Ensure continuous audit readiness through routine monitoring, quality reviews, documentation oversight, and performance improvement activities.
· Lead responses to regulatory reviews, audits, monitoring visits, and corrective action plans, ensuring timely implementation and sustained compliance.
· Partner with the Regulatory Affairs and Quality teams to identify trends, mitigate organizational risk, and implement sustainable operational improvements.
4. HCBS Performance Management & Accountability
· Monitor HCBS operational performance through established dashboards, reports, and performance measures.
· Ensure managers regularly review productivity, utilization, staffing, compliance, quality, and service delivery metrics.
· Use data to identify trends, address performance challenges, and implement improvement actions.
5. Leadership & People Management
· Lead, coach, mentor, and develop managers and supervisors, fostering leadership capacity and accountability throughout the organization.
· Build a high-performing, collaborative culture grounded in professionalism, integrity, continuous learning, and operational excellence.
· Establish clear expectations, monitor performance, provide regular coaching and feedback, and address performance concerns promptly through coaching, corrective action, and development planning.
· Develop leaders who effectively manage people, operations, and change while creating engaged, accountable, and customer-focused teams.
6. Financial & Operational Management
· Develop, monitor, and manage departmental budgets and operational performance.
· Partner closely with the Financial Controller and Chief Operating Officer to monitor expenditures, utilization, purchased services, productivity, and financial performance.
· Ensure responsible stewardship of public resources while balancing consumer needs, regulatory requirements, and financial sustainability.
7. Program Development & Community Partnerships
· Identify opportunities to strengthen, expand, and enhance Home and Community-Based Services through operational improvements, innovative service models, and strategic partnerships.
· Support the planning, implementation, and evaluation of new programs and initiatives that advance organizational goals and respond to the evolving needs of older adults, individuals with disabilities, and caregivers.
Requirements:· Registered Nurse (RN/BSN) or Licensed Clinical Social Worker (LCSW) Healthcare Administration, Public Health, Gerontology, or related field required; Master of Science in Nursing (MSN) or Licensed Independent Clinical Social Worker (LICSW) strongly preferred. Equivalent leadership experience in LTSS may be considered.
· Minimum five years of progressive leadership experience in Long-Term Services and Supports (LTSS), Home and Community- Based Services (HCBS), Aging Services, Disability Services, Home Care, Care Management, Managed Care, or related community-based programs.
· Master's degree preferred.
· Experience working in a unionized environment preferred.
· Proficiency with Microsoft Office, SharePoint, data dashboards, and electronic documentation systems (WellSky preferred).
