Ambulatory Clinical Pharmacy Specialist - Pulmonary
POSITION SUMMARY :
Under the direction of the Clinical Manager, the Ambulatory Clinical Pharmacy Specialist optimizes medication therapy and patient outcomes by preventing and resolving drug-related problems. This role functions under a collaborative practice agreement, evaluating patients and modifying therapy in accordance with federal, state, and institutional regulations.
The specialist develops a clinical practice across ambulatory pulmonary sites, collaborates with interdisciplinary partners, and serves as a drug therapy expert in pulmonary specialty medications. Responsibilities include direct patient care, education, quality improvement, formulary management, research, and training of pharmacists, residents, and students.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Clinical Care & Patient Management
- Provide patient-centered pharmacotherapy for interstitial lung disease, sarcoidosis and other pulmonary conditions requiring complex specialty medications
- Develop, document, and implement individualized plans of care using evidence-based guidelines and protocols
- Evaluate, monitor, and modify drug therapy, including prescribing or discontinuing medications under collaborative practice agreements
- Order, review, and interpret laboratory studies to guide therapy adjustments
- Monitor treatment response, adverse effects, and medication adherence
- Identify, manage, and resolve drug-related problems in collaboration with physicians
- Provide patient education and counseling for new and ongoing therapies
- Coordinate immunizations and infusion center medication ordering
- Refer patients to emergency or alternate services when clinically indicated
Documentation & Transitions of Care
- Document clinical assessments, encounters, and counseling in the electronic medical record
- Provide transition-of-care services, including medication reconciliation and post-discharge follow-up calls
- Communicate significant or atypical findings to primary providers
Medication Access & Operations
- Support prior authorization processes, denials, and copay assistance programs
- Supervise and provide direction to the pharmacy liaison (certified pharmacy technician)
- Serve as a liaison between clinical and distributive pharmacy services
Quality, Safety & Regulatory Compliance
- Identify opportunities to improve patient care and medication-use processes
- Lead and participate in quality improvement initiatives
- Assist with collection, analysis, and presentation of patient outcomes data
- Collaborate with stakeholders to ensure compliance with accreditation and regulatory requirements (e.g., URAC, Joint Commission)
Formulary & Informatics
- Participate in formulary review and management, including monographs, MUEs, and medication guidelines
- Contribute to the development and maintenance of order sets and clinical workflows within EPIC
- Serve as a clinical resource for formulary decisions and workflow design
Education, Teaching & Professional Development
- Precept APPE PharmD students, PGY-1 and PGY-2 residents
- Provide in-service education to pharmacists, nurses, physicians, and other healthcare professionals
- Participate in resident orientation and cross-training
- Maintain current knowledge of pulmonary therapies through continuing education (minimum of 5 disease-related hours annually)
- Engage in research, writing, and scholarly activities as appropriate
Additional Responsibilities
- Participate in institutional committees
- Provide clinical support for pulmonary and other specialty disease priorities as needed
JOB REQUIREMENTS
EDUCATION AND EXPERIENCE:
- Work requires at a minimum a Bachelor's Degree in Pharmacy from an accredited college of pharmacy and at least five years of pertinent work experience; or a Bachelor's Degree in Pharmacy from an accredited college of pharmacy and post-graduate training, with at least three years of pertinent work experience; or a Doctorate Degree in Pharmacy from an accredited college of pharmacy with post-graduate training or at least two years of pertinent work experience.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
- Work requires licensure to practice pharmacy in the state of Massachusetts by the hire date. Work requires substantial interpersonal skills towards our customers (particularly patients, physicians and nurses) and fellow employees of the hospital.
- Must be eligible for credentialing as a licensed independent provider practicing under a collaborative drug therapy management agreement.
Compensation Range:
$112,000.00- $162,500.00This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
