Minimum Requirements

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent professional degree (required).
• Current State License as a Physical Therapist in MA or compact/licensure that qualifies to practice in MA (required to start assignment).
• Proof of current CPR certification (required).
• Current driver’s license, reliable transportation, and proof of auto insurance (required).
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting (required).
• Previous home health experience (preferred).
• Experience with neuromuscular, neurological, and orthopedic treatment planning and durable medical equipment knowledge (preferred).
• Ability to perform physical demands of field work including lifting, bending, standing and prolonged walking (required).
• Ability to read, write, and speak fluent English (required).

Assignment Details

• Facility Type: Home Health.
• Shift Name: Other.
• Shift Pattern: 5x8.
• Start Date: Assignment begins April 2026 and extends through August 2026.
• Role: Provide in-home physical therapy services, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary team.
• Clinical Requirements: Must perform patient evaluations, instruct on assistive devices, order rehabilitative equipment as needed, and apply infection control/universal precautions.
• Travel Requirement: Must be able to travel locally to patient homes and work a flexible schedule.
