In this role, you’ll provide direct support, manage services, and supervise team members to ensure that every person we serve receives consistent, high-quality, and person-centered care.

What You’ll Do:
  • Support and coordinate services for individuals with developmental disabilities, ensuring their goals, preferences, and needs are reflected in every aspect of their support.
  • Develop and implement ISP goals , ensuring documentation and follow-through in collaboration with each person’s support team.
  • Advocate and communicate regularly with families, guardians, DDS, healthcare providers, and community partners.
  • Respond to emergencies and provide on-call support as needed.
  • Ensure compliance with state regulations and CHS policies, including safety, health, medication administration, and documentation standards.
  • Promote community inclusion by developing natural supports and helping individuals access medical, dental, and wellness services.
  • Participate in quality initiatives , team meetings, and continuous improvement efforts across programs.

Requirements:

What You Bring:
  • Associate’s Degree in a related field required; Bachelor’s Degree preferred.
  • Minimum of 2 years’ experience supporting individuals with developmental disabilities.
  • At least 1 year of supervisory experience in human services.
  • Strong documentation and communication skills.
  • Ability to multitask, problem-solve, and lead with compassion and professionalism.
  • Valid driver’s license (MA, NH, RI) and reliable transportation required.
  • Must be able to lift 30 lbs and meet the physical demands of the role.
