RN Healthcare Case Manager – Up to $55/hr
Are you a compassionate and experienced registered nurse passionate about making a real difference in patients' lives? We are seeking a dedicated RN Healthcare Case Manager to provide personalized, coordinated care to individuals with complex healthcare needs. In this vital role, you will act as a clinical guide—empowering members through education, advocacy, and resource coordination—helping them achieve their health goals every day. This is a unique opportunity to contribute meaningfully while working remotely in a supportive, dynamic environment.
Key Responsibilities:
- Conduct comprehensive clinical assessments to evaluate patient needs and develop tailored care plans
- Coordinate services across multidisciplinary teams to ensure seamless, continuous care delivery
- Educate members and their families on treatment plans, health management, and adherence strategies
- Monitor patient progress, adjusting care strategies as needed to meet changing health requirements
- Maintain compliance with case management principles, regulatory standards, and NCQA requirements
- Document all interactions and care plans accurately in healthcare databases, ensuring organizational and regulatory standards are upheld
Requirements:
- Minimum of 2 years of RN experience in inpatient, outpatient, or other healthcare settings
- Active RN licensure in Massachusetts
- Proficiency in electronic health record documentation systems
- Excellent verbal communication skills and comfort working via phone
- Must reside in Massachusetts; this is a fully remote role within the state
Nice-to-Have Skills:
- Prior case management or care management experience
- Experience working with diverse populations and individuals with complex needs
- Knowledge of social determinants of health and strategies to address care disparities
Preferred Education and Experience:
- Bachelor's degree in Nursing (BSN) preferred
- Certifications such as CCM (Case Management Certification), CDE (Certified Diabetes Educator), or Wellcoach are advantageous
