Baylor Nurse Job Description & Expectations

Position Summary: The Baylor Nurse is responsible for delivering high-quality patient care during weekend shifts, managing a caseload of home care visits, and maintaining effective communication with the care team to ensure seamless patient management.

Key Responsibilities:

 Conduct 40 patient visits over the weekend, including both AM and PM visits, ensuring timely and thorough care delivery.

 Remain accessible and responsive via Connectix and phone throughout the shift to handle patient calls, coordinate care, and communicate with on-call personnel.

 Maintain consistent contact with the on-call team to provide updates and receive guidance as needed.

 Follow the agency’s missed visit protocol diligently to ensure all scheduled visits are completed or appropriately reported.

 Submit a comprehensive end-of-shift report after completing Sunday PM visits to document care provided and any issues encountered.

 Monitor and respond to weekly reports received every Friday, ensuring any relevant patient care concerns are addressed promptly.

 Request weekend reports every Friday morning via the designated weekend report group to prepare for upcoming patient visits.

 For Element Care patients, promptly escalate any clinical concerns to the Element Care on-call nurse to ensure continuity and quality of care.

Qualifications:

 Current LPN -or- RN licensure with experience in home care preferred.

 Strong organizational and communication skills.

 Ability to manage a high-volume caseload efficiently and effectively over weekend shifts.  Proficiency in using Connecteam or similar communication platforms.

 Commitment to following established protocols and maintaining accurate documentation.

Pay: $40.00 - $45.00 per hour
