Per Diem Certified Nursing Assistant (CNA)
As a Per Diem Certified Nursing Assistant (CNA), you can either assist in daily routine or episodic daily nursing aide care to adults with brain injury on a per diem basis. Your work will be in a clinically intensive program that is tailored to meet the needs of a unique specialized population who have been diagnosed with brain injuries and complex health needs. In your role you will work together with the people we serve, their families, friends, employers, and other support staff to transform their lives.
Per Diem C.N.A. duties include administering medication under the Medication Administration Plan (MAP) guidelines, assisting individuals to become more a part of their communities, and helping the program to meet the highest quality standards. You will employ rehabilitation values, principles and techniques, including skills training to help individuals served overcome barriers and to live more independently.
Responsibilities
The essential job duties/responsibilities of the position are included in but not limited to the information listed below:
- Provides assistance with activities of daily living according to each client's/persons served treatment plan that includes, but is not limited to, community membership, community relationships, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, passive range of motion exercises, as needed or ordered by primary care provider.
- Responsible for a variety of daily tasks with clients/persons served. This includes rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, and other related tasks.
- Provides role model and support to clients/persons served. Assists individuals in becoming fully integrated into their communities.
- Takes and documents client's/person's served temperature, pulse, respirations, blood pressure, skin care, toileting, and/or passive range of motion exercises as ordered by primary care clinician.
- Implements and documents individual treatment support plans.
- Stores, administers, documents, and disposes of client's/persons served medication consistent with Medication Administration Program (MAP) requirements, funding source regulations and company policy, if applicable. Ensures program compliance with Vinfen MAP protocols.
- Transports, assists, and accompanies clients/persons served to medical, dental, and other appointments as directed by shift/program manager/administrator.
- Creates an environment in both the client's/person's served room and common area that is safe, clean, welcoming, and respectful to individuals, families, and neighbors. Ensures that the environment meets all safety and regulatory standards.
- Acts as an advocate for clients/persons served.
- Assesses and reports any changes in client's/person's served medical condition or mental status to supervisor.
- Participates in trainings, staff meetings, and other meetings, as requested.
- Works effectively and safely with assistive equipment that includes, but is not limited to, wheelchairs, Hoyer lifts, and other medical equipment. Maintains client's/persons served medical equipment and supplies that includes, but is not limited to wheelchairs, walkers, and Hoyer lifts. Works effectively and safely with medical equipment, that includes, but is not limited to, glucometers, nebulizers, and oxygen.
- Documents services provided to clients/persons served, as required by Vinfen Policy.
- Completes all service delivery, MAP, and administrative documentation in a thorough and timely manner.
- Contributes to program's quality improvement goals and objectives.
- Uses crisis intervention techniques when necessary. Accesses on-call back up when needed. Completes incident documentation, as needed.
- Ensures maintenance and safety of physical site. Oversees and participates in the daily living and cleaning of the program, inside and outside with clients/persons served. This includes cooking, meal planning, grocery shopping, general housekeeping, lawn mowing, snow shoveling, and garbage removal.
- Adheres to all company and divisional and program specific policies and procedures, including safe work practices, worker's compensation, universal precautions, and exposure control. Comply with accreditation and licensing standards, as well as regulations.
- Perform other related duties, as required.

Qualifications
Typical Requirements: One to two years of job-specific experience in human services field preferred. Experience working with individuals with brain injury preferred.
Preferred /Required Education: A high school diploma or equivalent is required.
Driving Requirements: May be called on to drive for Vinfen using either a Vinfen van or personal vehicle. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least one year, and must be able to pass a driver's screening background check, which is subject to annual checks. If driving, must maintain a safe driving record. If using a personal vehicle, must possess and maintain adequate insurance.
Physical Effort: Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift. Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
Ability to assist clients with tasks of daily living.
Ability to remain in a stationary position 50% of the time as needed. Ability to bend, reach, file, sit, stand, and move around the facility.
Ability to speak, hear, and communicate with clients, staff, and external representatives.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. For positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.
