Physical Therapist – Home Health
The Physical Therapist provides evaluations and skilled therapy services in the home setting to support patient recovery, mobility, and independence.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide and document physical therapy treatments.
- Monitor patient progress and modify care as needed.
- Educate patients and caregivers on exercises and safety.
- Maintain accurate clinical documentation.
- Collaborate with physicians and interdisciplinary care teams.
Requirements
- Active Physical Therapist license in the state of MA
- Minimum 1 year of PT experience preferred.
- Current BLS certification.
