Job Title: Registered Nurse (RN) – Home Health

Location: South Dennis, Massachusetts 02660
Assignment Duration: 13 weeks (with possible extension)
Schedule: 40 hours per week
Shift: Day Shift

The Home Health Registered Nurse (RN) is responsible for delivering high-quality nursing care in a home care setting. This includes conducting comprehensive assessments of patients’ physiological, psychosocial, and environmental conditions, along with reviewing health history to ensure safe and effective care. The RN develops, implements, and manages individualized care plans aligned with case management goals.

• Perform comprehensive patient assessments at the time of admission
• Develop, implement, and regularly update individualized care plans
• Identify patient needs and coordinate appropriate interventions
• Utilize HHRG values to determine visit frequency and interdisciplinary service needs
• Maintain HHRG values and outcomes aligned with national benchmarks
• Participate in interdisciplinary case conferences and evaluate care effectiveness
• Provide instruction and supervision to Home Health Aides every 14 days with proper documentation
• Monitor patient conditions and promptly report significant changes to physicians or care providers
• Maintain accurate, timely, and detailed clinical documentation
• Obtain and clarify physician orders for all care services
• Coordinate patient discharges, including documentation and communication with care teams
• Refer patients to appropriate community resources
• Complete all required documentation and administrative tasks on time
• Perform additional duties as assigned

• Active Registered Nurse (RN) license in Massachusetts
• Successful completion of pre-employment medication test
• Minimum 1 year of nursing experience required (or 6 months with BSN)
• Current CPR certification

• Bachelor of Science in Nursing (BSN)
• Prior home health or home care experience
• IV therapy experience
• Strong computer and clinical documentation skills
