JOB SUMMARY: Under the direct supervision of the Shelter Program Manger the Case Manager is responsible for providing case management, advocacy, support, and referrals to supportive community services to prepare individuals who are currently in emergency shelter for the goal of attaining more suitable housing. Case Management services include initial service needs assessment, engagement strategies, linkages to other resources, and assistance in accessing supports necessary for each client to transition to housing. Case manager must perform her job in a manner that enhances client self-esteem, offers a model of successful behaviors and supports learning. The job is performed in a residential setting, and therefore requires flexible scheduling to meet contract requirements and client needs. It will require evenings, night and weekend assignments.
Essential Functions of Position:
- Conduct in-depth assessments regarding immediate client concerns and/or barriers to housing
- Coordinate assessment and service delivery for Home Base resources.
- Participate in on site and off site training and meetings as directed.
- Case Manager will monitor and track efforts and outcomes regarding service delivery and housing retention.
- Assists participants through problem solving, direct support, intervention, de-escalation and mediation as necessary.
- Ensures participants adherence to shelter policies and procedures.
- Obtain resources and services as requested and/or needed to address barriers to housing and economic stabilization.
- Assist client with transitions supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services needed.
- Meet with shelter clients regularly to evaluate and document progress.
- Enter all required data in a timely and accurate manner into the HMIS database.
- Attend all required data-related training.
- Works collaboratively with the Re-Housing Case Manager to facilitate the goals of more suitable housing options.
- Attend case management meetings and assigned staff training.
- Ensure compliance with program/department, agency and/or funder requirements, as well as, FII policies & procedures.
- Other duties as assigned.
Knowledge, Skills and Abilities
- Basic understanding of coaching and mentoring practices and skills
- Cultural competency and ability to establish effective professional relationships with diverse group of clients.
- Knowledge of homelessness, housing subsidies, and other housing issues.
- Strong mediation, negotiation, verbal and written communication skills.
- Ability to handle a variety of assignments and have the flexibility to work some evenings if necessary.
- Strong organizational skills and high level of attention to detail.
- Excellent Microsoft Office skills/ETO

QUALIFICATIONS: Minimum educational requirements, Bachelor's Degree in Counseling, Social Work, Psychology, Human Services or other related discipline, Master’s degree preferred. A minimum of two years of work experience, Life experience, and demonstrated work experience with families experiencing homelessness, and crisis intervention are essential. According to the contract, this position must be filled by a female.
