✅ What You'll Do as a Home Health LPN

• Provide direct nursing care to patients in their homes across assigned territories
  * Administer medications, wound care, IV therapy, and other treatments
  * Educate patients and caregivers on disease processes, medications, and home care techniques
  * Collaborate closely with RN Case Managers and the full interdisciplinary care team
  * Ensure accurate and timely clinical documentation

What You'll Need to Join as a Home Health LPN:

• Active LPN license in the state of Massachusetts
• At least 2 years of nursing experience (1 year of home health preferred)
• Reliable transportation and a valid driver's license
